Discharge planning begins upon admission. Once you have made the commitment to begin your path of Recovery, we plan ways in which to help you succeed. We are confident in your success and begin planning for your discharge from the safe and regimented environment of Snug Harbor to a life of quality living using the tools gained in rehabilitation.
Discharge planning is a joint task involving both the client and the counselor. Clients gain valuable information during treatment and rehabilitation. Planning discharge will help you develop ways to use the information you have gained and to continually maintain your specific goals set in treatment.
Snug Harbor will contact you the day after discharge.
Snug Harbor will contact you one week after discharge.
Snug Harbor will contact you one month after discharge.
Snug Harbor will contact you once a month for 3 to 12 months after discharge. The purpose of this follow up is to ensure you are following the discharge plan and that you continue your journey of recovery.
